    This study is a descriptive research study conducted systematically analyze the nursing needs of patients hospitalized in integrated nursing care service wards on intentional nursing rounds to provide a basis for providing patient-centered nursing care.
    The study subjects included 188 adult patients 19 years and older who were hospitalized at a tertiary hospital in U Metropolitan City who had no communication problems and had been hospitalized in the integrated nursing care service wards for at least three days. The data were collected from August 13 to November 14, 2024 using a structured questionnaire. The collected data were analyzed using IBM SPSS Statistics 24.0 program and the prioritized nursing care needs using Importance-Performance Analysis (IPA), The Locus for Focus (LF) Model, and Borich Needs Analysis.

    The key findings from the study are as follows:

    1. Patients' perceived importance of nursing care was highest for communication (4.16±0.82 points) and pain (4.08±0.78 points), and the lowest for excretion (2.79±1.12 points). In terms of perceived performance, where patients perceive that nursing care services were performed, pain (3.86±0.90 points) and communication (3.82±0.98 points) scored the highest, while excretion (2.47±1.13) scored the lowest.

    2. Analysis of the difference between importance and perceived performance showed that importance was significantly higher than perceived performance for most nursing care service items. In particular, the largest difference was found in the item 'Nurses inform that they will make another round in an hour' with a score of 0.06 (p<0.001).
    3. Combining IPA, LF model, and Borich needs results showed that 'insomnia relief measures' was ranked as a top need priority across all three analysis methods. In addition, 'listening to patients' questions and concerens,' 'checking in on current status and needs,' and 'checking for recent changes or new symptoms' in the communication area also had high priorities.
    4. When analyzing the differences in nursing care needs for intentional nursing rounds by age, we found that patients under 65 had higher needs for 'application of cold/hot pack and assessment of skin condition' and 'pain relief measures,' while patients 65 and older had greater needs for 'item rearrangement,' 'checking the item locations,' and 'checking current status and needs.'
    5. Comparison of the difference in nursing care needs among surgery patients showed that the patients who did not undergo surgery showed a high need for 'application of cold/hot pack and assessment of skin and pain relief measures,' while surgery patients showed a higher need for 'checking the wearing status of protective and assistive gears.'
    6. When comparing the difference in nursing care needs according to the experience of hospitalization in the integrated nursing care service wards, patients with no hospitalization experience had higher needs for 'assistance with diaper changes,' and 'checking for pain,' while the patients with more experience had higher needs for 'application of cold/hot pack and assessment of skin and pain relief measures,' 'managing various drainage tubes,' 'checking current status and needs,' and 'pain relief measures.'
    7. As a result of comparing the difference in needs based on whether a caregiver was present, patients without caregivers had higher needs for 'communication areas,' while the patients with caregivers had higher needs for 'bedsore prevention measures,' 'item rearrangement,' 'managing various drainage tubes,' and 'checking for pain.'

    This study systematically identified the priorities of nursing care needs and proposed the effective design and implementation plan for intentional nursing rounds, highlighting the need for customized nursing care strategy to realize patient-centered care. In addition, it also recommended the need for follow-up studies that reflect a variety of wards and patient severity outside of the integrated nursing care service wards. The results of this study will serve as an important basis for improving the quality of nursing care and enhancing patient satisfaction.

    간호·간병통합서비스 병동 간호사의 의도적 간호 순회에 대한 환자의 간호 요구도 김 소 현 지도교수 문 성 미 울산대학교 대학원 간호학과 본 연구는 간호·간병통합서비스 병동에 입원한 환자들을 대상으로 의도적 간호 순회에 대한 간호 요구도를 체계적으로 분석하여 환자 중심 간호 제공을 위한 기초자료를 마련하고자 수행된 서술적 조사연구이다. 연구 대상은 U 광역시 소재 상급종합병원에 입원한 만 19 세 이상의 성인 환자 188 명으로, 의사소통에 문제가 없고 간호·간병통합서비스 병동에 3 일 이상 입원한 환자들이었다. 자료 수집은 2024 년 8 월 13 일부터 11 월 14 일까지 구조화된 설문지를 사용하여 이루어졌다. 수집된 자료는 IBM SPSS Statistics 24.0 프로그램을 이용하여 분석하였으며, 중요도-수행도 분석(IPA), The Locus For Focus(LF) 모델, Borich 요구도 분석을 활용하여 간호 요구도의 우선순위를 도출하였다. 본 연구의 주요 결과는 다음과 같다. 1. 환자들이 인식하는 간호 중요도는 소통(4.16±0.82점)과 통증(4.08±0.78점) 영역에서 가장 높게 나타났으며, 배설(2.79±1.12점)영역은 가장 낮은 중요도 를 보였다. 환자들이 간호 서비스를 수행 받았다고 인식하는 수행 인식도에서 는 통증(3.86±0.90점)과 소통(3.82±0.98점)영역이 높은 점수를 기록했으며, 배설(2.47±1.13점)영역이 가장 낮았다. 2. 중요도와 수행 인식도의 차이를 분석한 결과, 대부분의 간호 서비스 항목에서 중요도가 수행 인식도보다 유의하게 높았다. 특히, ‘간호사가 1시간 후에 다시 순회를 올 것이라고 알리는 것’ 항목에서 0.06점으로(p<0.001) 가장 큰 차이 가 나타났다. 3. IPA, LF 모델, Borich 요구도 결과를 종합한 결과, ‘불면 완화 조치’는 세 가지 분석 방법 모두에서 요구도 최우선 항목으로 선정되었다. 또한, 소통 영역의 ‘환자의 질문 및 걱정을 경청’, ‘현재 상태 및 요구사항 확인’, ‘최근의 변화나 새 로운 증상 확인’ 항목 역시 높은 우선순위를 차지하였다. 4. 연령에 따른 의도적 간호 순회에 대한 간호 요구도 차이를 비교하여 분석한 결 과, 65세 미만 환자는 ‘냉찜질/온찜질 적용 및 피부 상태 사정’과 ‘통증 완화 조 치’에 높은 요구도를 보였고, 65세 이상 환자는 ‘물품 재배치’, ‘물품 위치 확인’, ‘현재 상태 및 요구사항 확인’에 더 큰 요구도를 나타냈다. 5. 수술 여부에 따른 간호 요구도 차이를 비교한 결과, 수술을 받지 않은 환자는 ‘냉찜질/온찜질 적용 및 피부 상태 사정’과 ‘통증 완화 조치’에 높은 요구도를 보였으며, 수술을 받은 환자는 ‘보호대 및 보조기구 유지 상태 확인’에 대한 요 구도가 높았다. 6. 간호·간병통합서비스 병동 입원 경험에 따른 간호 요구도 차이를 비교한 결과, 입원 경험이 없었던 환자는 ‘기저귀 교환 보조’, ‘통증 여부 확인’에 요구도가 높 았고, 경험이 많은 환자는 ‘냉찜질/온찜질 적용 및 피부 상태 사정’, ‘각종 배액 관 관리’, ‘현재 상태 및 요구사항 확인’, ‘통증 완화 조치’에 대한 요구도가 높게 나타났다. 7. 보호자 상주 여부에 따라 요구도 차이를 비교한 결과, 보호자가 없는 환자는 ‘소통 영역’에 대한 요구도가 높게 나타났으며, 보호자가 상주하는 환자는 ‘욕창 예방 조치’, ‘물품 재배치’, ‘각종 배액관 관리’, ‘통증 여부 확인’에 대한 요구도가 높게 나타났다. 본 연구는 간호 요구도의 우선순위를 체계적으로 파악하여 의도적 간호 순회의 효과적 설 계와 실행 방안을 제안하였으며, 환자 중심 간호를 실현하기 위한 맞춤형 간호 전략의 필요 성을 강조하였다. 또한, 간호·간병통합서비스 병동 외의 다양한 병동 및 환자의 중증도를 반영한 후속 연구의 필요성을 제언하였다. 본 연구의 결과는 간호 서비스의 질적 향상과 환 자 만족도 증진을 위한 중요한 기초자료로 활용될 수 있을 것이다.
