[연구배경] 계절성 인플루엔자는 여전히 세계적으로 질병 부담이 크며, 코로나19 이후 우리나라의 인플루엔자 감염률은 다시 증가 추세이다. 계절성 인플루엔자 바이러스의 감염 확산 및 인플루엔자 팬데믹을 예방하기 위해 청장년층의 예방접종률 향상을 위한 노력이 필요하지만, 관련 연구는 부족하다. 본 연구는 앤더슨 의료이용 행동모형을 적용하여 청장년층의 계절성 인플루엔자 예방접종 행태의 결정요인을 탐구하였다.
[연구방법] 연구자료는 2022년 지역사회건강조사 원시자료에서 서울에 거주하는 65세 미만 성인의 자료를 이용하였다(n=14,535). 독립변수는 소인요인(성별, 연령), 가능요인(소득수준, 교육수준, 혼인상태), 필요요인(만성질환 유무, 현재 흡연 여부, 고위험 음주 여부, 신체활동 실천 여부, 코로나19 백신접종 수준)이다. 종속변수는 인플루엔자 예방접종 여부이다. 통계분석은 성별과 연령별로 계층화하여 이항 로지스틱 회귀분석을 실시하였다.
[연구결과] 연구 결과에 따르면 남성은 미혼이고(OR=0.611), 만성질환이 없으며(OR=0.799), 현재 흡연자이고(OR=0.822), 고위험 음주를 하고(OR=0.744). 코로나19 백신접종 수준이 낮을수록(OR=0.532) 계절성 인플루엔자 예방접종 가능성이 작았다. 여성의 경우, 교육수준이 낮고(OR=0.840), 미혼이고(OR=0.449), 만성질환이 없고(OR=0.778), 코로나19 백신접종 수준이 낮을수록(OR=0.540) 계절성 인플루엔자 예방접종을 할 가능성이 작았다.
[결론] 흡연과 고위험 음주 등 건강 위험행동을 하는 사람들은 감염 위험이 크지만 예방접종률은 낮은편이다. 이러한 집단을 대상으로 맞춤형 메시지를 제공할 필요가 있다. 청년층에게 전반적인 건강 인식 제고를 위한 교육을, 중장년층에게는 예방접종을 포함한 건강위험 행동 개선 프로그램을 제공하는 등 연령대별 특성을 고려하여 계절성 인플루엔자 예방접종률 향상 전략을 모색해야 한다.

Determinants of Seasonal Influenza Vaccination
Objectives: Seasonal influenza continues to pose a significant health burden globally, and. the infection rate in Korea is rising once again following the COVID-19 pandemic. There is an urgent need to enhance vaccination rates among young adults to mitigate the spread of seasonal influenza viruses and prevent future pandemics. However, research in this area is limited. This study investigates the factors influencing seasonal influenza vaccination behavior among young adults by utilizing the Anderson behavioral model.
Methods: The study utilized data from adults under age of 65 residing in Seoul, derived from the raw data of the 2022 Community Health Survey (n=14,535). Independent variables include predisposing factors (such as gender and age), enabling factors (including income level, education level, and marital status), and need factors (such as the presence of chronic diseases, current smoking status, high-risk drinking behavior, physical activity levels, and COVID-19 vaccination status). The dependent variable is influenza vaccination. For statistical analysis, binomial logistic regression was conducted, stratifying the data by gender and age.
Results: Studies have demonstrated that men who are single (OR=0.611), have no chronic diseases(OR=0.799), are current smokers (OR=0.822), consume alcohol at high-risk levels(OR=0.744), and/or have a lower level of COVID-19 vaccination(OR=0.532) are less likely to receive the seasonal influenza vaccination. For women, factors such as a lower education level(OR=0.840), being unmarried(OR=0.449), the absence of chronic diseases(OR=0.778), and a lower level of COVID-19 vaccination (OR=0.540) contribute to a decreased likelihood of receiving the seasonal influenza vaccination.
Conclusions: People who engage in health risk behaviors, such as smoking and high-risk drinking, are at and increased risk of infection but tend to have low vaccination rates. Therefore, it is essential to deliver tailored massages to these populations. Strategies to enhance influenza vaccination rates should take into account the specific characteristics of each age group. For instance, efforts should focus on raising overall awarness among young people and addressing health risk behaviors, including vaccination, among middle-aged individuals.
